Based on your new patient scenario provided by your faculty, develop a care plan. The objective and subjective assessment data should be pulled from the patient scenario you received. Use the care plan template provided below.
You will need to provide:
Assessment of data relevant to the nursing diagnosis, distinguishing between the Subjective and Objective data
One NANDA-approved nursing diagnosis is based on the assessment data included in the patient scenario and must provide the related factors and AEB factors. No “Risk for” diagnosis accepted.
One short-term goal using the SMART guidelines
One long-term goal using the SMART guidelines
A minimum of 3 nursing interventions for each nursing diagnosis; remember MEATA.
Include scientific rationale for each nursing intervention
Cite the source for rationale in APA format
Evaluate each goal and give evidence of whether the goal was reached
