USE THE LOGIC MODEL FOR HIGH BLOOD PRESSURE TO COMPLETE THIS ASSIGNMENT.
Purpose:
Now that we have a Logic Model and know where the project intends to go, it is essential to build a detailed plan of how each intervention will be executed so that all “inputs” are used effectively.
Actions:
Reminder: You will be creating the implementation plan independently because you will be implementing the project independently. You should use the plan as a road map or structured list to help guide the work in the “do phase” of your project in the next module.
Implementation plan template.docx
Independently (not as a group), create your one-week (module 4) Implementation Plan. Use this when you implement your adapted program next week.
The preparation phase should outline all of the detailed steps that you need to do to “prepare” to implement your project in module 4.
Do you need to create any materials, remind participants, or practice the intervention?
Most dates for this section should be module 3 and the beginning of module 4.
The implementation phase should outline all of the steps that you must do to execute your adapted intervention in module 4.
How can you ensure that you have all of the materials and are collecting all of the process indicator data on time?
Dates for this section should be in module 4.
The post-implementation phase should cover all of the steps that you need to do to collect your post-implementation data and analyze your results.
What post-implementation data collection and analysis steps need to be created?
In most projects, the dates should be at the end of module 4 for data collection and module 5 for post-implementation analysis.
Include audience-tailored materials that you will use during the implementation and leave with the participants as takeaways.
Did you create a presentation? Did you create a poster? Did you put together a video? Include the materials you need to have to make module 4’s implementation be successful.
Note: This is a living document that may be updated in the future as you further define later sections of your plan or external factors out of your control impact the scope of your implementation.
Consider:
For each day of the week – who does what, by when, with what resources, what objectives are being achieved, and what needs to be prepared?

There is an example of how to fill out the implementation plan on the template form. YOU MUST COMPLETE THE TEMPLATE (for who, put Subject) and CREATE PRE AND POST SURVEY PLANS, TEXT MESSAGES TO SEND OUT, ANYTHING THAT IS NEEDED TO IMPLEMENT THE EXPERIMENT.
I’ve also attached a reduction plan document to guide you with what you need to complete this assignment
