Background:
Mr. Samuel Rodriguez is a 68-year-old Hispanic male with a primary diagnosis of type 2 diabetes mellitus (DM). He also suffers from hypertension and hyperlipidemia. His medical history includes diabetic retinopathy, peripheral neuropathy, and diabetic nephropathy. He is overweight with a BMI of 32. Mr. Rodriguez lives alone after his wife passed away last year. He has two adult children who visit occasionally but live in another state. He speaks limited English, which poses a challenge for communication and understanding his medical care. He relies on public transportation for his medical appointments.
Current Medications:
Metformin 1000mg PO bid, Lisinopril 20mg PO qd, Atorvastatin 40mg PO qd, Lantus insulin 20 units subcutaneously qhs, Aspirin 81mg PO qd.
Social History:
Mr. Rodriguez is retired and spends most of his time at home. He enjoys cooking traditional Hispanic meals but struggles to maintain a balanced diet due to his diabetes. He has a pet dog that provides him companionship. He has a few close friends in the neighborhood whom he occasionally socializes with. He attends a weekly diabetes support group at the local community center.
